Coronavirus Appointment Information

What Patients Need to Know Before Arriving:

  1. Before your appointment, we will email you the forms for your visit. Please complete these forms prior to your arrival and return them to the office via email, mail, or fax. If you can’t do that, please bring completed forms with you to your visit.

  2. Please bring your mobile phone with you so that we can call or text you at your appointment time. This is important.

  3. Please do not arrive more than 10 minutes before your scheduled appointment time.

  4. Please bring a face mask or face covering and wear it at all times. This is mandatory.

 

What Will Happen Upon Arrival for an Appointment:

  1. Patients are asked to stay in the car and call us on arrival. We will ask you some screening questions before you enter the facility.

  2. When we are ready for you to enter the facility, we will call or text you to let you know we are ready to meet you at the front entrance.

  3. We will register patients and visitors entering the facility in case we need to do contact tracing.

  4. We may take your temperature upon arrival. If you have a fever, we will reschedule.

  5. Only the patient will be allowed to enter the facility, unless the patient is a child or requires assistance, in which case, one person may accompany the patient. That person will be required to wear a face covering at all times.

  6. All persons entering the facility will be asked to sanitize their hands upon arrival.

  7. We are asking patients to maintain social distancing and we are monitoring common areas to make sure that social distancing guidelines are respected.

 

Actions We Are Taking at the Office:

  1. Our office is disinfected thoroughly on a daily basis and common areas are disinfected throughout the day.

  2. We will wear face masks and maintain social distancing throughout your visit, except at points in the examination when the doctor will have to get closer to you.

  3. We will clean all instruments used for patient examination and all surfaces in the room between appointments.

  4. We are offering telehealth visits for non-vision threatening conditions, including vision therapy.

  5. We encourage patients to reschedule if they have flu-like symptoms.
