What Patients Need to Know Before Arriving:

1. Before your appointment, we will email you the forms for your visit. Please complete these forms prior to your arrival and return them to the office via email, mail, or fax. If you can’t do that, please bring completed forms with you to your visit.

2. Please do not arrive more than 10 minutes before your scheduled appointment time.

3. We have parking available behind our office building.

What We Ask of Our Patients:​

1. Be mindful of other patients in the office. 

2. Our waiting area has limited space. If you're accompanying a child in therapy or waiting for another patient, feel free to utilize other areas of the building. This is a great opportunity to run errands or take a walk while you wait.
