Appointment Information

Mask Policy Update:

In accordance with the updated guidance from the Centers for Disease Control (CDC) on February 25th, 2022, we have adjusted our mask policy effective on Monday March 7th, 2022.

Following is a list of the three levels of COVID severity and the appropriate mask requirement. We will monitor the CDC website at the beginning of each week and post the level for each week based on the CDC website.

Low (Green) – Masks are optional, unless staff or patient has symptoms of any illness.

Medium (Yellow) - Masks are optional, unless staff or patient has symptoms of any illness. Staff may still wear a mask if working closely with a patient.

High (Red) - Masks required for Patients and Staff (unless 2 years old or younger)

If at any point a patient feels more comfortable if the staff wears masks, we are happy to accommodate.

We appreciate your help in following the mask policy throughout the pandemic

to help keep each other healthy!

What Patients Need to Know Before Arriving:

  1. Before your appointment, we will email you the forms for your visit. Please complete these forms prior to your arrival and return them to the office via email, mail, or fax. If you can’t do that, please bring completed forms with you to your visit.

  2. Please do not arrive more than 10 minutes before your scheduled appointment time.

  3. Please bring a face mask or face covering.

 

What We Are Asking of Our Patients:

  1. Be mindful of other patients in the office. Do our best to maintain social distancing measures while in the waiting area.

  2. We do not have a lot of space in our waiting area. If you have a child in therapy or are waiting on another patient in an appointment, it would be best to wait in your car if possible. Also a good time to run an errand or take a walk while waiting. 

 

Actions We Are Taking at the Office:

  1. Our office is disinfected thoroughly on a daily basis and common areas are disinfected throughout the day.

  2. We will clean all instruments used for patient examination and all surfaces in the room between appointments.

  3. All of our employees have been vaccinated.

  4. We encourage patients to reschedule if they have flu-like symptoms.
