Coronavirus Appointment Information

What Patients Need to Know Before Arriving:

  1. Before your appointment, we will email you the forms for your visit. Please complete these forms prior to your arrival and return them to the office via email, mail, or fax. If you can’t do that, please bring completed forms with you to your visit.

  2. Please do not arrive more than 10 minutes before your scheduled appointment time.

  3. Please bring a face mask or face covering.

 

What We Are Asking of Our Patients:

  1. In accordance with Ohio Optometric Association and CDC recommendations, masks are required for all visits.

  2. Be mindful of other patients in the office. Do our best to maintain social distancing measures while in the waiting area.

  3. We do not have a lot of space in our waiting area. If you have a child in therapy or are waiting on another patient in an appointment, it would be best to wait in your car if possible. Also a good time to run an errand or take a walk while waiting. 

 

Actions We Are Taking at the Office:

  1. Our office is disinfected thoroughly on a daily basis and common areas are disinfected throughout the day.

  2. We will wear face masks and maintain social distancing throughout your visit, except at points in the examination when the doctor/therapist will have to get closer to you.

  3. We will clean all instruments used for patient examination and all surfaces in the room between appointments.

  4. All of our employees have been vaccinated.

  5. We encourage patients to reschedule if they have flu-like symptoms.
